ABSTRACT Objective: There is no consensus as to the best criterion for the evaluation of metabolic syndrome (MS), impairing the estimation of its prevalence. This study aims to compare MS estimates using three recommended definitions for adolescents based on a cross-sectional study nested in the Consortium of Brazilian Birth Cohorts in São Luís, Maranhão. Subjects and methods: A total of 2,515 adolescents aged between 18 and 19 years were evaluated. The criteria of International Diabetes Federation (IDF) and National Cholesterol Education Program Panel III (NCEP-ATP) modified by Cook and cols. (2003) and De Ferranti and cols. (2004) defined SM. To compare the estimates of MS prevalence, the chi-square, Fisher's exact and Cohen's Kappa index tests were used. Results: Among the 2,064 participants evaluated in the final sample. The prevalence of MS ranged from 4.2% (95% CI: 3.3-5.1) to 10.2% (95% CI: 8.8-11.4). When comparing the estimates of MS prevalence in the total sample and by sex, a statistically significant difference was observed. The agreement between the criteria ranged from 0.42 (CI 95%: 0.35-0.49) to 0.55 (CI 95%: 0.48-0.62) in the total sample, 0.33 (CI 95%: 0.24-0.42) to 0.59 (95%CI: 0.47-0.71) among boys and 0.39 (95% CI: 0.26-0.52) to 0.54 (95% CI: 0.44-0.64) among girls. Conclusion: Different criteria provide different estimates for the prevalence of MS in adolescents, reflecting the importance of establishing a consensus.

ABSTRACT Objective: To analyze different anatomical sites in the abdominal region, in order to determine the positional parameter that identifies a higher level of visceral adipose tissue (VAT) and confers a greater cardiometabolic risk. Materials and methods: This is a methodological study in which VAT was evaluated by ultrasonography (USG) in three anatomical sites in the abdomen, while the abdominal circumference (AC) was measured using seven different protocols. Additionally, the glycemic and lipid profile, C-reactive protein, and the presence of systemic arterial hypertension were evaluated. Results: One hundred and six individuals with an average age of 42 (36.8-46.2) years were included. The evaluation of the calibration of the ultrasound procedure for the analysis of VAT by intra- and inter-evaluators showed high reproducibility. The pattern of abdominal fat distribution differed between sexes, with higher mean VAT in males (p < 0.05) and higher mean SAT (subcutaneous adipose tissue) in females (p < 0.005). In the abdominal scan applied to women, higher levels of VAT and lower levels of SAT were observed in the narrower waist region, between the iliac crest and the last rib (p < 0.001). In males, the profile of adipose disposition along the abdomen was uniform (p > 0.05). Correlations between VAT measured by USG and cardiometabolic parameters were relatively stronger in the upper abdomen (p < 0.05). Conclusion: Women accumulate more VAT in the narrower waist region, while men accumulate VAT uniformly across the abdomen. There was relative superiority in predicting cardiometabolic risk in the upper abdomen for both sexes.

ABSTRACT Objective To identify the association between dietary patterns and nutritional status in adolescent freshmen at a public university in Northeastern Brazil. Methods In this cross-sectional study anthropometric variables, body composition and food intake were collected and assessed using the food frequency questionnaire. Dietary patterns were evaluated through factor analysis using the principal component extraction method. Results Two dietary patterns were identified: "Western" pattern, consisting of foods with high energy density and low nutritional value, and the "traditional Brazilian" pattern, with foods from Brazilian cuisine such as rice, beans, corn, roots and tubers, fruits, greens and vegetables. The multiple regression analysis revealed a negative association between the "traditional Brazilian" pattern and both excess weight and body fat in females. Conclusion A healthy dietary pattern with typical local cuisine foods can offer protection to health and should be encouraged.


RESUMO Objetivo Identificar a associação entre padrões alimentares e estado nutricional em adolescentes recém-ingressos em uma universidade pública do Nordeste brasileiro. Métodos Neste estudo transversal foram coletados dados antropométricos, composição corporal e consumo alimentar, verificado pelo questionário de frequência alimentar. Os padrões alimentares foram derivados através da análise fatorial pelo método de extração de componentes principais. Resultados Dois padrões alimentares foram identificados: o padrão "Ocidental", composto por alimentos de elevada densidade energética e de baixo valor nutricional, e o padrão "tradicional brasileiro", contendo alimentos da culinária brasileira como arroz, feijão, milho, raízes e tubérculos, frutas, verduras e legumes. A análise de regressão múltipla mostrou associação negativa entre o excesso de peso e de gordura corporal e o padrão alimentar "tradicional brasileiro" no sexo feminino. Conclusão Um padrão alimentar saudável e com alimentos típicos da culinária local pode conferir proteção à saúde, devendo ser incentivado.

Resumo Objetivo Analisar os componentes da Síndrome Metabólica (SM) e a racionalidade na adoção dos critérios utilizados para sua classificação em idosos por meio de uma revisão integrativa Método Trata-se de uma revisão integrativa da literatura nos idiomas português, inglês, espanhol e búlgaro com idosos ( idade ≥ 60 anos), nas bases de dados eletrônicas MEDLINE - via PubMed, Embase, Web of Science e Scopus, sem restrição de tempo de publicação e desenho de estudo, no período de agosto 2022 a janeiro de 2023. Utilizaram-se os descritores controlados do Medical Subject Headings (MeSH) e Descritores em Ciências da Saúde (DEcS) "idoso", "idoso 80 ou mais", "síndrome metabólica", "prevalência", "componente" e "critério". Foram excluídos estudos que não tratavam da temática, publicações não disponíveis na íntegra e que não atenderam à pergunta norteadora. Resultados Após a aplicação dos critérios de elegibilidade, dos 1340 estudos encontrados, foram incluídos quatorze estudos na revisão integrativa. Foram identificados sete critérios de avaliação da SM na população idosa e que a maioria dos estudos utilizou dois dos sete critérios, sendo o critério de IDF citado sete vezes nos quatorze artigos escolhidos e o do NCEP - ATP III mencionado oito vezes. Conclusões Embora as buscas tenham reunido diferentes estudos acerca do assunto, foi possível concluir que os critérios que têm sido propostos para a síndrome metabólica precisam ser revisados e terem seus pontos de corte definidos de acordo com a população estudada.


Abstract Objective To analyze the components of Metabolic Syndrome (MetS) and the rationale for adopting the criteria used for its classification in older individuals through an integrative review. Method an integrative review of the literature in Portuguese, English, Spanish and Bulgarian involving older people (age ≥60 years) on the electronic databases MEDLINE - via PubMed, Embase, Web of Science and Scopus, without restriction on publication date or study design, was conducted from August 2022 to January 2023. The Medical Subject Headings (MeSH) and Health Sciences Descriptors (DEcS) controlled descriptors "elderly", "elderly 80 or older", "metabolic syndrome", "prevalence", "component" and "criterion" were used. Studies that were off-topic, unavailable in full and that failed to answer the guiding question were excluded. Results Application of the eligibility criteria led to the retrieval of 1340 studies, of which 14 were included in the integrative review. Seven sets of criteria were identified for evaluating MetS in the older population were identified and most studies used two of these criteria. The IDF criterion was cited 7 times and the NCEP - ATP III criterion 8 times in the 14 article reviewed. Conclusions Although the searches retrieved different studies on the subject, the results suggest the criteria for MetS should be revised, with cut-off points defined according to the population studied.

ABSTRACT Objective To assess linear growth and weight gain in infants with suspected cow's milk protein allergy with gastrointestinal manifestations, seen at a gastropediatrics clinic. Methods A retrospective cohort study conducted with demographic, clinical, anthropometric and dietary information on 84 infants first seen between 2015 and 2018 and followed-up for six months. Stature-for-age, weight-for-age, and body mass index-for-age in z-scores were evaluated according to the cut off points established by the World Health Organization in 2006. Accelerated growth or catch-up was considered a gain ?0.67 in the z-score of the referred indices, evaluated at 3 and 6 months. Results Median age at baseline was 4.0 months and 88.1% of the infants were already in diet exclusion. Regarding the anthropometric evaluation short stature frequency was 15.5% and the underweight frequency was 8.3% and 3.6% respectively based on the weight-for-age and body mass index-for-age indices. High recovery growth was observed during the follow-up period but was not considered catch up. In boys, the gains in weight-for-age and body mass index-for-age were significant (p=0.02 and p=0.01 respectively) and close to the threshold that characterizes the catch up, 0.58 and 0.59, respectively. In girls, significant gains in stature-for-age and weight-for-age (0.38 and 0.37 respectively, p=0.02 for both) were observed. Conclusion Infants with suspected cow's milk protein allergy with gastrointestinal manifestations should have early access to specialized nutritional counseling to avoid exposure to allergenic food and control allergy symptoms, thereby avoiding malnutrition and ensuring adequate nutritional recovery.


RESUMO Objetivo Avaliar o crescimento linear e o ganho de peso de lactentes com suspeita de alergia à proteína do leite de vaca, com manifestações gastrointestinais, atendidos em um ambulatório de gastropediatria. Métodos Estudo de coorte retrospectivo, com informações demográficas, clínicas, antropométricas e dietéticas de 84 lactentes que iniciaram atendimento entre 2015 e 2018 e foram acompanhados durante seis meses. Foram avaliados os índices estatura/idade, peso/idade e índice de massa corporal/idade em escore-z, segundo os pontos de corte da Organização Mundial de Saúde de 2006. Considerou-se crescimento acelerado (ou catch-up growth) um ganho ?0,67 escore-z nos referidos índices, avaliados em três e seis meses. Resultados No baseline, a mediana de idade foi 4,0 meses e 88,1% dos lactentes já estavam em dieta de exclusão. A frequência de baixa estatura foi de 15,5% e a de baixo peso foi de 8,3% e de 3,6% segundo os índices peso/idade e índice de massa corporal/idade, respectivamente. Houve elevado crescimento de recuperação durante o período de acompanhamento, mas que não configurou catch up. Nos meninos, ganhos no peso/idade e índice de massa corporal/idade foram significantes (p=0,02 e p=0,01) e próximos do limiar que caracteriza catch up: 0,58 e 0,59, respectivamente. Nas meninas, foram observados ganhos significantes na estatura/idade e peso/idade, de 0,38 e 0,37 (p=0,02 para ambos). Conclusão Demonstrou-se que lactentes com suspeita de alergia à proteína do leite de vaca, com manifestações gastrointestinais, deveriam ter acesso precoce ao aconselhamento nutricional para evitar exposição ao alimento alergênico, controlar sintomas e, assim, evitar a desnutrição ou garantir recuperação nutricional adequada.

Abstract As obesity has reached epidemic proportions and given the current recognition of central adiposity as an important cardiometabolic risk factor, several researchers have focused on developing and validating predictive indexes and equations to evaluate Visceral Adipose Tissue (VAT). This study evaluates the applicability of the Visceral Adiposity Index (VAI) for predicting cardiometabolic risk in individuals treated in a hospital In the northeast region of Brazil. The VAT was evaluated by computed tomography (CT) and the VAI was calculated through specific equations for each gender. The sample involved adult and elderly patients of both genders followed up in a cardiology outpatient clinic. The following cardiometabolic parameters were collected: fasting glycemia, glycated hemoglobin, lipid profile, C-reactive protein (CRP) and uric acid. The simple linear regression was used to evaluate the explanatory power of the VAI in relation to the volume of VAT determined by CT. The predictive capacity of VAI in relation to the volume of VAT determined by CT was 25.8% (p=0.004) for males and 19.9% (p<0.001) for females. VAI correlated strongly with the triglyceride (TG) (p<0.001) and TG/high-density lipoprotein (HDL) ratio (p<0.001) and inversely correlated with HDL (p<0.001). Moreover, VAI showed low correlation with the following variables: abdominal circumference, total cholesterol, low density lipoprotein, fasting glycemia, and glycated hemoglobin (p<0.05). VAI was associated with variables considered as cardiometabolic risk factors, but exhibited a low predictive capacity regarding the volume of VAT determined by CT. Thus, caution is recommended in its use in Brazilian individuals.


Resumo Em razão de a obesidade ter alcançado proporções epidêmicas e dado ao atual reconhecimento da adiposidade central como um importante fator de risco cardiometabólico, diversos pesquisadores têm se dedicado em desenvolver e validar índices e equações preditivas para avaliar o Tecido Adiposo Visceral (TAV). Este estudo avaliou a aplicabilidade do Índice de Adiposidade Visceral (IAV) como preditor de risco cardiometabólico em indivíduos atendidos em um hospital no nordeste brasileiro. O TAV foi avaliado por tomografia computadorizada (TC) e o IAV foi calculado através de equações específicas para cada sexo. A amostra envolveu pacientes adultos e idosos de ambos os sexos acompanhados no ambulatório de cardiologia. Os seguintes parâmetros cardiometabólicos foram coletados: glicemia de jejum, hemoglobina glicada, perfil lipídico, proteína C-reativa e ácido úrico. Regressão linear simples foi empregada para avaliar o poder explicativo do IAV em relação ao volume de TAV determinado por TC. A capacidade preditiva do IAV em relação ao volume de TAV determinado pela TC foi de 25,8% (p=0,004) para o sexo masculino e 19,9% (p<0,001) para o sexo feminino. O IAV se correlacionou fortemente com as variáveis TG (r=0,916, p< 0,001) e TG/HDL (r=0,952, p<0,001) e inversamente com o HDL (r=-0,441, p<0,001), além disso, apresentou baixa correlação com as variáveis: circunferência abdominal, colesterol total, lipoproteína de baixa densidade, glicemia de jejum e hemoglobina glicada (p<0,05). O IAV associou-se com variáveis consideradas fatores de risco cardiometabólico, porém exibiu baixa capacidade preditiva em relação ao volume de TAV determinado pela TC, sendo recomendada cautela em sua utilização em indivíduos brasileiros.

ABSTRACT

A difusão de padrões alimentares ditos "não saudáveis" são fatores de risco independentes para as Doenças Crônicas Não Transmissíveis. Considerando essa importância, foi realizada uma revisão integrativa com o objetivo de analisar as evidências científicas atuais que retratam a qualidade da dieta do idoso no Brasil e seus fatores associados. O levantamento bibliográfico abrangeu publicações brasileiras, no período de 2011 a 2020, nas bases de dados LILACS e Pubmed, sendo selecionados ao final da pesquisa documental 18 documentos originais. Os resultados apontaram que a dieta da população idosa brasileira encontra-se monótona e pobre em nutrientes, sendo um potencial fator de risco para o desenvolvimento e/ou agravamento de doenças. Deve-se, portanto, enfatizar a importância do desen-volvimento de novos estudos, principalmente nas regiões Norte, Nordeste e Centro-oeste, a fim de subsidiar o planejamento de políticas públicas voltadas à alimentação e nutrição desse grupo populacional. Ações concentradas nesse sentido poderão contribuir, sobremaneira, com a promoção da saúde e melhoria da qualidade de vida da população envelhecida.(AU)


The propagation of unhealthy eating patterns is an independent risk factor for Chronic Noncommunicable diseases. Given this importance, an integrative review was conducted, and has as aim to analyze the current scientific evidence that portrays the quality of the elderly's diet in Brazil and its associated factors. The biblio-graphic survey covered Brazilian publications, from 2011 to 2020, in the LILACS and Pubmed databases; 18 original documents were selected after the review. The articles showed that the diet of the elderly Brazilian is monotonous and poor in nutrients, being a poten-tial risk factor for the development and/or worsening of diseases. Therefore, it is necessary to emphasize the importance of developing new es, mainly in the North, Northeast and Midwest regions for the purpose of subsidizing the planning of public policies for food and nutrition for this population group. Targeted actions in this direc-tion may contribute, significantly, with the promotion of health and improvement of the quality of life of elderly.(AU)

ABSTRACT Objective: Cardiovascular diseases represent the main cause of death in chronic kidney disease (CKD). We aimed to evaluate the prevalence and association of the hypertriglyceridemia-waist phenotype (HWP) and visceral adiposity index (VAI) with cardiometabolic risk factors (CR) in patients with CKD on hemodialysis (HD). Materials and methods: The study is based on a cross-sectional design with 265 HD patients in two cities in northeastern Brazil. The VAI was calculated considering the variables body mass index (BMI), waist circumference (WC), triglycerides (TG) and high density lipoprotein cholesterol (HDL-c). HWP was defined as the concomitant elevation of WC and TG. The Poisson Regression Model with robust variance estimation was adjusted considering a hierarchical approach for explanatory variables. Prevalence ratios (PR) were also estimated. The level of significance adopted was 5%. Results: In our study HWP and VAI prevalence's were 29.82% and 58.49%, respectively. In the final model, there was an association between VAI and female gender (PR = 1.46; p < 0.0001) and high body fat (% BF) (PR = 1.33; p < 0.0019). HWP was associated with females (PR = 1.80; p = 0.002), alcohol consumption (PR = 1.58; p = 0.033), obesity (PR = 1.89; p = 0.0001), high %BF (PR = 1.76; p = 0.012) and reduced HDL-c (PR = 1.48; p = 0.035). Conclusion: The HWP stood out as the association with more CR factors, representing a promising method for tracking cardiometabolic risk in HD patients, mainly female.

Resumo Introdução A síndrome metabólica (SM) é o agrupamento de fatores de risco associados à adiposidade central e à resistência à insulina. Objetivo O objetivo desta pesquisa foi analisar a prevalência da SM e os fatores associados em idosos de uma comunidade do município de Recife, Pernambuco. Método Estudo transversal com 166 idosos de ambos os sexos com idade igual ou superior a 60 anos, residentes na Comunidade dos Coelhos, no município de Recife. Para o diagnóstico da SM, foi utilizado o critério do NCEP-ATPIII. A análise dos dados foi efetuada com a utilização do programa estatístico SPSS para Windows, versão 13.1. Na comparação entre proporções, foram utilizados o teste de qui-quadrado de Pearson e o teste exato de Fischer. A razão de prevalência foi utilizada como medida de efeito considerando a SM como variável dependente. Resultados A prevalência de SM encontrada foi de 38,3%. Os fatores associados à SM após ajustes foram: a idade, na faixa etária 60-69 anos (44,4%, p = 0,047), o sexo feminino (43,1%, p = 0,056) e o excesso de peso (57,3%, p < 0,001). Conclusão A SM apresentou uma alta prevalência na população idosa avaliada, sendo associada às características demográficas e ao estado nutricional.


Abstract Background Metabolic syndrome (MS) is a clustering of risk factors associated with central adiposity and insulin resistance. Objective This study aimed to analyze the prevalence of MS and associated factors in older adults living in a subnormal urban cluster located in the city of Recife, state of Pernambuco, Brazil. Method A cross-sectional study was carried out with 166 older adults of both sexes aged ≥60 years residing in the Coelhos Community in Recife. MS was diagnosed through the NCEP/ATP III criteria. Data analysis was performed using the SPSS 13.1 statistical software. The Pearson's chi-squared test and the Fisher's exact test were used to compare the proportions. Prevalence ratio was used as an effect measure considering MS as dependent variable. Results Prevalence of MS was 38.3%. The factors associated with MS after adjustments were being 60-69 years old (44.4%, p=0.047), female (43.1%, p=0.056), and overweight (57.3%, p<0.001). Conclusion High prevalence of MS was observed in the older population evaluated, being associated with demographic characteristics and nutritional status. The group of older adults assessed should be provided with nutritional education and motivation to the practice of physical activities.

ABSTRACT Objective: evaluate the relationship between visceral/subcutaneous adipose tissue (VAT/SAT) and glomerular filtration rate (GFR). Methods: A case series study was conducted with 146 male and female adult outpatients at a hospital in Northeast Brazil. VAT and SAT were quantified using computed tomography and GFR was estimated using the formula proposed by the CKD Epidemiology Collaboration. The conceptual model also considered socio-demographic, clinical, anthropometric and lifestyle variables. Results: Females accounted for 71.9% of the sample and mean age was 52.5±13.2 years. Mean body mass index indicated obesity in both sexes (men:30.4±5.9 kg/m2; women: 31.6±6.1 kg/m2). For the same mean age and BMI, men had more VAT and a higher VAT/SAT ratio. Mean GFR was similar between sexes and within the normal range. Simple linear regression analysis revealed that 21.8% of the reduction in GFR in males could be explained by the VAT/SAT ratio (p=0.002). Among females, both VAT alone and the VAT/SAT ratio were predictors of GFR reduction (r2=4.8%, p=0.025 and r2=5.3%, p=0.019, respectively). Conclusion: Mean VAT and VAT/SAT ratio were compatible with abdominal obesity in both sexes and were related to a reduction in GFR.


RESUMEN Objetivo: Evaluar la relación entre el tejido adiposo visceral (TAV) y subcutáneo (TAS) con la tasa de filtración glomerular. Métodos: Estudio tipo serie de casos, en 146 pacientes adultos de ambos sexos, atendidos en un ambulatorio de un hospital de referencia en el Nordeste brasileño. El TAV y el TAS se cuantificaron por tomografía computadorizada y la tasa de filtración glomerular (TFG) estimada por la fórmula del grupo Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI). El modelo conceptual también consideró variables sociodemográficas, clínicas, antropométricas y de estilo de vida. Resultados: Pacientes con edad promedio de 52,5 ± 13,2 años y el 71,9% de sexo femenino. El promedio del IMC en ambos sexos se encuentra en el rango de obesidad (hombres= 30,4 ± 5,9 kg/m2 vs mujeres= 31,6 ± 6,1 kg/m2). Para un mismo promedio de edad e IMC, los hombres presentaron mayor TAV y mayor razón TAV/TAS que las mujeres. El promedio de la TFG fue similar entre los sexos y se encuentra en el rango normal. A través de regresión lineal simple, se evidenció que, en el sexo masculino, la disminución de la TFG puede explicarse en el 21,8% por la razón TAV/TAS (p=0,002). En el sexo femenino, tanto el TAV aislado como la razón TAV/TAS fueron predictores de disminución de la TFG (r2= 4,8%; p=0,025 e r2= 5,3%; p=0,019), respectivamente. Conclusión: Se evidenciaron valores muy elevados de los parámetros antropométricos de obesidad abdominal y promedio de TAV y de la razón TAV/TAS compatible con obesidad visceral en ambos sexos, siendo que estos dos últimos parámetros estuvieron relacionados al descenso de la TFG.

ABSTRACT

A deficiência de vitamina A e os distúrbios neurológicos em idosos constituem problemas de Saúde Pública que devem ter mais atenção por parte das políticas públicas do idoso institucionalizado, devido à sua gravidade e diversidade de causas. Assim, este estudo objetiva avaliar os fatores de risco associados à deficiência de vitamina A em idosos institucionalizados da cidade de João Pessoa-PB. Trata-se de um estudo transversal e analítico, em que foram selecionados 105 idosos, ≥60 anos, de cinco Instituições de Longa Permanência para Idosos. Avaliaram-se parâmetros sociodemográficos, clínicos, antropométricos, de retinolemia, de consumo de vitamina A, de estresse oxidativo e inflamação. Encontrou-se uma prevalência de deficiência de vitamina A (<1,05 micromol/L) em 30,5% da amostra (32 idosos). Em relação ao consumo de alimentos-fonte de vita- mina A, 68,6% (72 idosos) da amostra consumiam os de origem animal numa frequência maior do que 3x/semana, enquanto que os alimentos de origem vegetal eram consumidos por 64,8% (68 idosos) da amostra, nessa mesma frequência. A capacidade antioxidante total foi quantificada em 10±17 % e o malondialdeído 3,3±1µmol; a alfa glicoproteína ácida em 38±33 mg/dL (p>0,05). Os indivíduos esquizofrênicos possuem 7,75 (p =0,00; IC 95% = 2,56-26,59) vezes mais chances de serem deficientes de retinol sérico, independen- temente de outros fatores de risco. Assim, existe a necessidade de adoção de medidas de saúde pública e um maior controle desses fatores de risco que podem piorar/afetar a qualidade de vida dessa população idosa institucionalizada.(AU)


Vitamin A deficiency and neurological disorders in the elderly are Public Health problems that should be given more attention by public policies for institutionalized elderly people, due to their severity and diversity of causes. Thus, this study aims to assess the risk factors associated with vitamin A deficiency in institutionalized elderly in the city of João Pessoa-PB. This is a cross-sectional and analytical study, in which 105 elderly, ≥60 years old, from five Long Term Care Institutions for the Elderly were selected. Sociodemographic, clinical, anthropometric, retinolemia, vitamin A consumption, oxidative stress and inflammation parameters were evaluated. A prevalence of vitamin A deficiency (<1.05 micromol / L) was found in 30.5% of the sample (32 elderly). Regarding the consumption of vitamin A source foods, 68.6% (72 elderly) of the sample consumed those of animal origin at a frequency greater than 3x/week, while foods of plant origin were consumed by 64.8% (68 elderly) of the sample, at the same frequency. The total antioxidant capacity was quantified at 10 ± 17% and malondialdehyde 3.3 ± 1 µmol; alpha acid glycoprotein at 38 ± 33 mg / dL (p> 0.05). Schizophrenic individuals are 7.75 (p = 0.00; 95% CI = 2.56-26.59) times more likely to be serum retinol deficient, regardless of other risk factors. Thus, there is a need for the adoption of public health measures and greater control of these risk factors that can worsen/affect the quality of life of this institutionalized elderly population.(AU)

ABSTRACT Objective: to assess the effects of a problematization educational intervention to promote healthy habits in elderly people with diabetes. Methods: a randomized clinical trial conducted with 202 individuals drawn for the intervention group and the control group. The intervention consisted of problematization educational activities on a monthly basis for over six months. The control group participated in conventional monitoring at the health unit. Data were collected through semi-structured interviews before and after the intervention, in addition to laboratory tests. Results: after the intervention, when compared to the control group, there was an increase in knowledge about the disease (p<0.001), positive attitude towards self-care (p=0.011), physical activity (p=0.020), diet variety (p=0.002), and lower consumption of oils and fats (p<0.05). Conclusion: the intervention performed has a beneficial effect to promote healthy habits.


RESUMEN Objetivo: evaluar los efectos de una intervención educativa problemática para la promoción de hábitos saludables en personas mayores con diabetes. Métodos: ensayo clínico aleatorizado, realizado con 202 individuos elegidos para dos grupos: intervención y control. La intervención consistió en problematizar las actividades educativas grupales, mensualmente, durante seis meses. El grupo de control participó en el monitoreo convencional en la unidad de salud. Los datos fueron recolectados a través de entrevistas semiestructuradas antes y después de la intervención, además de las pruebas de laboratorio. Resultados: después de la intervención, un aumento en el conocimiento sobre la enfermedad (p<0.001), una actitud positiva hacia el autocuidado (p=0.011), actividad física (p=0.020), variedad de dieta (p=0.002) y menor consumo de aceites y grasas (p<0.05), en comparación con el grupo de control. Conclusión: la intervención realizada tiene un efecto beneficioso para la promoción de hábitos saludables.


RESUMO Objetivo: avaliar os efeitos de uma intervenção educativa problematizadora para a promoção de hábitos saudáveis em idosos com diabetes. Métodos: ensaio clínico randomizado, realizado com 202 indivíduos sorteados para dois grupos: intervenção e controle. A intervenção consistiu em atividades educativas grupais problematizadoras, mensalmente, ao longo de seis meses. O grupo controle participou do acompanhamento convencional na unidade de saúde. Os dados foram coletados mediante entrevistas semiestruturadas antes e após a intervenção, além da realização de exames laboratoriais. Resultados: após a intervenção, foi evidenciado aumento do conhecimento sobre a doença (p<0,001), atitude positiva para o autocuidado (p=0,011), prática de atividade física (p=0,020), variedade da dieta (p=0,002) e menor consumo de óleos e gorduras (p<0,05), quando comparado ao grupo controle. Conclusão: a intervenção realizada tem efeito benéfico para a promoção de hábitos saudáveis.

RESUMO A Pesquisa Translacional é interdisciplinar e está apoiada em três pilares: pesquisa de bancada (básica), leito (aplicações clínicas) e comunidade (aplicações nos sistemas de saúde). O estudo, baseado nos cinco estágios da Pesquisa Translacional, resgatou o histórico da deficiência de vitamina A e da cegueira nutricional no Brasil (T0); o caminho da descoberta científica à escolha da intervenção - suplementação vitamínica (T1); a avaliação da eficácia da intervenção candidata por ensaio randomizado e controlado (T2); a avaliação da implementação e da cobertura na prática (T3); e a avaliação do impacto da intervenção (T4). Para verificar o impacto, aplicou-se a estatística superior de Wald, visando identificar quebras estruturais ao longo da série histórica da mortalidade geral de crianças entre 6 e 59 meses de idade. Para a região Nordeste, que apresentou a maior cobertura programática, o modelo sinalizou três quebras - agosto/1984, junho/1994 e maio/2006 -, nas quais foram estimadas reduções de 10%, 17% e 23%, respectivamente, na ocorrência mensal de óbitos infantis. O processo para a construção do conhecimento sobre a deficiência desta vitamina, a escolha da intervenção, a aplicação deste conhecimento no estabelecimento do Programa Nacional de Suplementação de Vitamina A e a avaliação do seu impacto configuram um exemplo de Pesquisa Translacional em saúde coletiva.


ABSTRACT Translational Research is interdisciplinary and supported by three pillars: bench (basic investigation), bedside (clinical applications) and community (applications in healthcare systems). The study, based on the five stages of translational research, reviewed the history of vitamin A deficiency and nutritional blindness in Brazil (T0); the pathway from scientific discovery to intervention choice - vitamin supplementation (T1); an assessment of the candidate intervention efficacy via randomized controlled trial (T2); the assessment of implementation and coverage in practice (T3) and the intervention impact assessment (T4). To estimate the impact, we applied Wald superior statistics to identify structural breaks along the time series of general mortality of children between 6 and 59 months of age. In the Northeast, region that presents the largest program coverage, the model pointed to three breaks: August/1984, June/1994 and May/2006, in which we estimated reductions of 10%, 17% and 23%, respectively, in the monthly incidence of children´s death. The process to construct knowledge about vitamin A deficiency, the choice of the intervention, the knowledge translation to establish the National Vitamin A Supplementation Program and the evaluation of its impact, constitute an example of translational research in collective health.

Abstract Background: Systemic arterial hypertension is a substantial public health problem responsible for millions of deaths per year worldwide. However, little is known about the epidemiology of this disease in areas distant from large urban centers in Brazil. Such information is necessary to plan health promotion strategies. Objective: To estimate the prevalence of hypertension and determine its associated factors in adults residing in the semi-arid region of the state of Pernambuco, Northeastern Brazil. Method: This is a cross-sectional study conducted with a random sample of male and female adults. Individuals with systolic blood pressure ≥ 140 mm/Hg and/or diastolic blood pressure ≥ 90 mm/Hg and those who reported being under treatment with antihypertensive drugs were considered hypertensive. We collected data on demographic, socioeconomic, behavioral, and anthropometric characteristics, as well as health and nutrition. The statistical analysis used Pearson's chi-square test, the chi-square test for trend, and multivariate Poisson regression analysis. A p-value < 0.05 in the final model was considered indicative of statistical significance. Results: The sample consisted of 416 individuals, and the prevalence of hypertension was 27.4% (95%CI 23.2 - 32.0). In the final model, the independent predictors of hypertension were age of 40 years or older (p = 0.000), low economic class (p = 0.007), smoking (p = 0.023), overweight determined by the body mass index (p = 0.003), and reduced glucose tolerance/diabetes mellitus (p = 0.012). Conclusion: The prevalence of hypertension was high and related to important risk factors. Thus, prevention and control strategies are recommended.


Resumo Fundamento: A hipertensão arterial sistêmica (HAS) constitui grande problema de saúde pública, responsável por milhões de mortes por ano no mundo. Apesar disso e frente à necessidade de informações para o delineamento de estratégias de promoção da saúde, pouco se conhece sobre a epidemiologia da doença em regiões distantes dos grandes centros urbanos brasileiros. Objetivo: Estimar a prevalência e verificar os aspectos associados à HAS em adultos do sertão de Pernambuco, Brasil. Métodos: Estudo de delineamento transversal, com uma amostra aleatória de adultos de ambos os sexos. Foram considerados hipertensos aqueles que apresentaram pressão arterial sistólica ≥ 140 mmHg e/ou diastólica ≥ 90 mmHg, além daqueles que relataram estar em tratamento com anti-hipertensivos. Informações demográficas, socioeconômicas, comportamentais, antropométricas e de saúde e nutrição foram coletadas. Foram aplicados os testes do 2 de Pearson, c2 para tendência e a regressão multivariada de Poisson. No modelo final, foi considerada significância estatística quando p < 0,05. Resultados: A amostra foi composta por 416 indivíduos e a prevalência de HAS foi de 27,4% (IC95% 23,2 - 32,0). No modelo final, foram identificados como preditores independentes da HAS a faixa etária a partir dos 40 anos (p = 0,000), classe econômica baixa (p = 0,007), tabagismo (p = 0,023), excesso de peso pelo índice de massa corporal (p = 0,003) e adultos com tolerância à glicose diminuída/diabetes mellitus (p = 0,012). Conclusão: A prevalência de HAS é elevada e se relaciona com fatores de risco importantes, logo, são recomendáveis ações de prevenção e controle.

Abstract Objective: Evaluate the association between inflammatory process, adiposity, and vitamins A, D, and E in adolescents, according to gender. Methods: Cross-sectional study with adolescents aged 12-19 years old of both genders attending public schools in Recife. A questionnaire was used to collect data on socioeconomic level, lifestyle, and food intake of adolescents. Then, an anthropometric evaluation and a blood sampling were performed to analyze serum concentrations of α-1-acid glycoprotein, retinol, β-carotene, α-tocopherol, and 25-hydroxy-vitamin D. Results: The levels of α-1-acid glycoprotein were higher for abdominal obesity in both genders. Male adolescents with insufficient serum α-tocopherol levels had low levels of α-1-acid glycoprotein (p = 0.03) and an increased risk of 25-hydroxy-vitamin D and β-carotene deficiency in relation to total and abdominal fat; female adolescents had an increased risk of insufficient β-carotene with abdominal obesity (PR: 1.33; 95% CI: 1.2-1.5). Conclusion: Abdominal adiposity implies a higher risk of inflammation and causes different changes to the levels of fat-soluble vitamins according to gender.


Resumo: Objetivo: Avaliar a associação entre processo inflamatório, adiposidade e as vitaminas A, D e E em adolescentes, segundo o sexo. Métodos: Estudo transversal com adolescentes de 12 a 19 anos de ambos os sexos de escolas públicas de Recife. Foi utilizado um questionário para coleta de dados socioeconômicos, de estilo de vida e de consumo alimentar dos adolescentes. Em seguida, realizou-se a avalição antropométrica e coleta de sangue para análise das concentrações séricas de α-1-glicoproteína ácida, retinol, β-caroteno, α-tocoferol e 25-hidroxivitamina D. Resultados: Os níveis de α-1-glicoproteína ácida foram maiores na obesidade abdominal de ambos os sexos. Os meninos com níveis séricos insuficientes de α-tocoferol expressaram níveis reduzidos de α-1-glicoproteína ácida (p = 0,03) e apresentaram um maior risco de deficiência de 25-hidroxivitamina D e β-caroteno na adiposidade total e abdominal, enquanto as meninas mostraram maior risco de insuficiência de β-caroteno com a obesidade abdominal (RP 1,33; IC 95% 1,2-1,5). Conclusão: A adiposidade abdominal reflete maior risco de inflamação e causa alterações distintas nas concentrações das vitaminas lipossolúveis, de acordo com o sexo.

Resumo Objetivou-se avaliar o conhecimento sobre o diabetes, a atitude para o autocuidado e os fatores associados, por meio de estudo transversal, na linha de base de um ensaio clínico randomizado, com idosos diabéticos na atenção primária de saúde do Recife, Nordeste do Brasil. Utilizou-se o Diabetes Knowledge Scale (DKN-A) e o Diabetes Attitudes Questionnaire (ATT-19). Dos 202 idosos, 77,7% apresentaram conhecimento insuficiente sobre a doença, com destaque para a cetonúria, substituição de alimentos e desconhecimento das causas e dos cuidados com a hipoglicemia. Quanto à atitude, 85,6% tiveram ajustamento psicológico negativo em relação ao diabetes. O modelo de regressão logística mostrou que morar sozinho foi fator de proteção (OR = 0,24; IC95% 00,9-0,65; OR = 0,22; IC95% 0,07-0,71) e baixa escolaridade fator de risco (OR = 7,78; IC95% 3,36-18,01; OR = 13,05; IC95% 4,63-36,82) para conhecimento insuficiente e atitude negativa para o autocuidado, respectivamente. Os achados reforçam a necessidade de ações educativas interdisciplinares que incluam aspectos socioeconômicos, psicoemocionais e educacionais na gestão do diabetes com vistas à manutenção da autonomia e funcionalidade do idoso.


Abstract This study aimed to assess the knowledge about diabetes, the attitude for self-care and associated factors through a cross-sectional study, the baseline of a randomized clinical trial with elderly diabetic in primary health care in Recife, Northeastern Brazil. We used the Diabetes Knowledge Scale (DKN-A) and Diabetes Attitudes Questionnaire (ATT-19). Of the 202 elderly, 77.7% had insufficient knowledge of the disease, especially for ketonuria, food replacement and were unaware of the causes and care of hypoglycemia. As for attitude, 85.6% had a negative psychological adjustment for diabetes. The logistic regression model showed that living alone was a protective factor (OR = 0.24; 95% CI 0.09-0.65; OR = 0.22; 95% CI 0.07 to 0.71), and low education, a risk factor (OR = 7.78; 95% CI 3.36-18.01; OR = 13.05; 95% CI 4.63-36.82) for the insufficient knowledge and the negative attitude for self-care, respectively. The findings reinforce the need for interdisciplinary educational actions that include socioeconomic, psycho-emotional and educational aspects in diabetes management to maintain elderly autonomy and functionality.

ABSTRACT Objectives: To estimate the degree of variability of the waist circumference (WC) when obtained in different anatomical sites and compare the performance of the measurement sites as predictors of visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) and cardiometabolic abnormalities. Subjects and methods: Cross-sectional study involving 119 individuals with overweight (50.3 ± 12.2 years), in which six WC measurement sites were evaluated (minimal waist, immediately below the lowest rib, midpoint between the lowest rib and the iliac crest, 2 cm above the umbilicus, immediately above the iliac crest, umbilicus level), in addition to the VAT and SAT (quantified by computed tomography) and cardiometabolic parameters. Results: The differences between the measurements ranged from 0.2 ± 2.7 cm to 6.9 ± 6.7 cm for men, and from 0.1 ± 3.7 cm to 10.1 ± 4.3 cm for women. The minimum waist showed significant correlation with VAT (r = 0.70) and with a higher number of cardiometabolic parameters among men. Regarding women, the WC measurement showed high correlation with SAT and moderate correlation with VAT, not being found superiority of one measurement protocol in relation to the others when assessed the correlation with VAT and with cardiometabolic parameters. Conclusions: Greater variability between the measuring sites was observed among women. With respect to men, the minimum waist performed better as a predictor of VAT and cardiometabolic alterations.

Objetivo: O objetivo deste estudo foi investigar a associação do retinol sérico, proteína C-reativa ultra-sensível (PCR-us) e a ingestão de fibras alimentares em uma população de idosos hipertensos. Material e Métodos: Trata-se de um estudo transversal de base populacional com 170 idosos com idade entre 60 e 90 anos, de ambos os sexos, de uma cidade do Nordeste do Brasil. Para as análises bioquímicas, as concentrações de retinol sérico e PCR-us foram analisadas e um questionário quantitativo de frequência de alimentar auto-administrado foi coletado. Foram utilizados como critérios de inclusão: idosos de 60 a 90 anos que residiam na cidade de João Pessoa e hipertensão auto-relatada. Os critérios de exclusão foram indivíduos com transtornos neuropsiquiátricos e indivíduos que relataram suplementação de vitamina A, carotenóides ou suplementos de fibras. Os dados foram analisados com o teste de Wilcoxon, teste exato de Fisher e análise de regressão logística. Todas as análises estatísticas foram realizadas com o Software R Development Core Team. Resultados: Não foram observadas relações significativas entre hipertensão arterial e níveis séricos de retinol (p = 0,4325), níveis de PCR (p = 0,4104) e consumo de fibra alimentar (p = 0,0935). Com base na regressão logística, cada aumento de 1 unidade no índice de massa corporal (IMC) aumenta a probabilidade de hipertensão em 0,009545%. As outras variáveis não contribuíram para o modelo de regressão final. Conclusão: Considerando a alta prevalência de indivíduos hipertensos com valores adequados de retinol, não houve associação entre retinol sérico, níveis de PCR e ingestão de fibra. A maioria dos participantes apresentou níveis normais de PCR-us, que podem ter sido influenciados pela atividade antioxidante do retinol e pela ingestão inadequada de fibras. (AU)


Objective: To investigate the association between serum retinol, highlsensitivityC- reactive protein (hs-CRP) and dietary fiber intake in a population of hypertensive elderlies. Material and Methods: This was a cross-sectional population-based study with170 elderly people aged 60-90 years, of both sexes, from a city in northeastern Brazil. Biochemical analyses were performed, in which serum retinol and hs- CRP concentrations were analyzed, and a self-administered quantitative food frequency questionnaire was applied. Inclusion criteria were: 60-90 years old, living in the city of Joao Pessoa, and selfreported hypertension. Exclusion criteria were: individuals with neuropsychiatric disorders, and individuals who reported supplementation of vitamin A, carotenoids or fiber supplements. The data were analyzed using Wilcoxon test, Fisher's exact test and logistic regression analysis. All statistical analyses were performed with the R Development Core Team Software. Results: No significant relationships were observed between hypertension and serum retinol levels (p = 0.4325), hs-CRP levels (p = 0.4104) and dietary fiber intake (p = 0.0935). Based on logistic regression, each 1 unit increase in body mass index (BMI) increases the probability of hypertension by 0.009545%. The other variables did not contribute to the final regression model. Conclusion: Considering the high prevalence of hypertensive individuals with adequate retinolvalues, there was no association between serum retinol, CRP levels and fiber intake. The majority of participants had normal hs-CRP levels, which may have been influenced by the antioxidant activity of retinol and inadequate fiber intake. (AU)

RESUMEN La grasa visceral es una importante medida predictora de riesgo cardiometabólico, sin embargo, se ha sugerido que la razón entre los componentes visceral y subcutáneo (TAV/TAS ≥ 0,4) puede ser un indicador más apropiado para evaluar la predisposición para acumular grasa visceral (GV) y las alteraciones cardio-metabólicas. Objetivo: Evaluar la predisposición para acumular GV y su asociación con alteraciones cardio-metabólicas. Métodos: Estudio transversal con 147 individuos atendidos en un ambulatorio de un hospital en el Nordeste brasileño. Se evaluaron el TAV y el TAS por tomografía computarizada y se investigaron variables demgráficas, clínicas, antropométricas y factores de riesgo cardiometabólico. Resultados: El promedio de edad fue 52,7±13,2 años y se observó una elevada razón TAV/TAS de 0,79±0,29 en hombres y 0,54±0,22 en mujeres. Se correlacionó la razón TAV/TAS con perfil glucídico y puntuacion de calcio coronariano (PCC) (p< 0,05), mientras que el TAV aislado presentó correlación directa con colesterol no-HDL, triglicéridos (TG), razón TG/HDL, glicemia, hemoglobina glicosilada y PCC (p<0,05) e inversa con HDL-c (p= 0,001). Conclusiones: Se observó una elevada predisposición para la acumulación de TAV. La razón TAV/TAS presentó correlación con alteraciones cardiometabólicas, pero la concentración de TAV aislada se correlacionó con un número superior de parámetros, siendo en esta investigación un predictor más potente que la razón TAV/TAS para indicar esas alteraciones.


ABSTRACT Visceral fat is an important predictor of cardiometabolic risk, but evidence suggests that the ratio between visceral and subcutaneous fat (≥0.4) may be a more appropriate indicator to assess individual predisposition to accumulate visceral fat (VF) and predict cardiometabolic alterations. Objective: To evaluate the predisposition to accumulate VF and its association with cardiometabolic alterations. Methods: A cross-sectional study was conducted with 147 patients seen at a hospital in Northeast Brazil. VF and subcutaneous fat (SF) were evaluated by computed tomography and anthropometric and cardiometabolic risk factors were investigated. Results: The mean age was 52.7±13.2 years. Mean VF/SF ratio was high: 0.79±0.29 for men and 0.54±0.22 for women and was correlated with the glucose profile and coronary calcium score (CCA) (p< 0.05). VF was directly correlation with non-HDL cholesterol, triglycerides (TG), TG/HDL ratio, glycemia, glycated hemoglobin and CCA (p< 0.05) and inversely with HDL-c (p= 0.001). Conclusions: A high predisposition to VF accumulation was observed in the population. VF/SF ratio correlated with cardiometabolic alterations, but the concentration of isolated VF correlated with a higher number of parameters. In this investigation, VF alone was a more powerful predictor of cardiometabolic alterations than the ratio of VF/SF.

ABSTRACT

OBJETIVOS: Investigar a associação entre parâmetros antropométricos e hipertensão arterial sistêmica e identificar os melhores preditores antropométricos dessa condição em mulheres afrodescendentes, de comunidades remanescentes de quilombos. MÉTODOS: Estudo transversal realizado com mulheres quilombolas do Estado de Alagoas. Foram investigados pressão arterial, parâmetros antropométricos (índice de massa corporal, circunferência da cintura, razão cintura-quadril, razão cintura-estatura, índice de conicidade, gordura corporal), variáveis sociodemográficas, tabagismo e paridade. As associações entre parâmetros antropométricos e hipertensão arterial sistêmica foram averiguadas por meio da regressão de Poisson com ajuste robusto da variância. A capacidade desses parâmetros em predizer a presença de hipertensão arterial sistêmica foi analisada por meio de curvas ROC (Receiver Operating Characteristic). RESULTADOS: Foram avaliadas 1.553 mulheres, com idades entre 20 e 59 anos. A prevalência de hipertensão arterial sistêmica foi 35,8% e a de excesso de peso foi 48,5%. A presença de hipertensão arterial sistêmica associou-se a índice de massa corporal, circunferência da cintura, razão cintura-quadril, razão cintura-estatura e gordura corporal, mesmo após o ajuste para idade, classe socioeconômica e tabagismo. A partir das curvas ROC, foram encontrados os seguintes pontos de corte: índice de massa corporal ≥26,2 kg/m², circunferência da cintura ≥81,6 cm, razão cintura-quadril ≥0,84, razão cintura-estatura ≥0,54, índice de conicidade ≥1,20 e gordura corporal ≥35,4%. Gordura corporal, razão cintura-quadril e razão cintura-estatura apresentaram igual capacidade em predizer a hipertensão arterial sistêmica. CONCLUSÕES: Todos os indicadores de obesidade global e os de obesidade central, excetuando-se o índice de conicidade, associaram-se à hipertensão arterial sistêmica nessa amostra de mulheres afrodescendentes quilombolas. Os melhores preditores antropométricos de hipertensão arterial sistêmica foram porcentagem de gordura corporal, razão cintura-quadril e razão cintura-estatura. Essas medidas tiveram igual, embora baixo, poder discriminatório para a presença de hipertensão arterial sistêmica nessa população.


AIMS: To determine the association between anthropometric parameters and systemic arterial hypertension and to identify the best anthropometrics predictors of this disease in afro-descendant women from remaining quilombo communities. METHODS: A cross-sectional study was conducted with quilombola women from Alagoas State. Blood pressure, anthropometric parameters (body mass index, waist circumference, waist-to-hip ratio, waist-to-height ratio, conicity index, body fat), sociodemographic variables, smoking and parity were investigated. The associations between anthropometric parameters and systemic arterial hypertension were investigated using Poisson regression with robust variance adjustment. The ability of these parameters to predict the presence of systemic arterial hypertension was analyzed using Receiver Operating Characteristic (ROC) curves. RESULTS: A total of 1,553 women, aged between 20 and 59 years, were evaluated. The prevalence of systemic arterial hypertension was 35.8% and that of overweight was 48.5%. The presence of systemic arterial hypertension was associated with body mass index, waist circumference, waist-to-hip ratio, waist-to-height ratio, and body fat, even after adjusting for age, socioeconomic class, and smoking status. From the ROC curves, the following cutoff points were found: body mass index ≥26.2 kg/m², waist circumference ≥81.6 cm, waist-to-hip ratio ≥0.84, waistto-height ratio ≥0.54, conicity index ≥1.20 and body fat ≥35.4%. Body fat, waist-to-hip ratio and waist-to-waist ratio were equally able to predict systemic arterial hypertension. CONCLUSIONS: All indicators of global obesity and those of central obesity, except for the conicity index, were associated with systemic arterial hypertension in this sample of Afro-descendant quilombola women. Percentage of body fat, waist-to-hip ratio and waist-to-height ratio were the best anthropometric predictors of systemic arterial hypertension. These measures had equal, albeit low, discriminatory power for the presence of systemic arterial hypertension in this population.
